General: This is a well-developed, well-nourished, in no acute distress. HEENT: Normocephalic, atraumatic. Pharynx is without erythema or exudates. Neck exam: No carotid bruits. Strength is 5/5 with flexion and extension. Lungs: Clear to auscultation bilaterally. Cardiovascular: Regular rate and rhythm, without murmurs, rubs or gallops. Neurological exam: Mental status: The patient is alert and oriented to person, place and time. Speech is fluent. Follows commands well and gives a good history. Cranial nerves II through XII are intact bilaterally: -Visual fields are full to confrontation. -Pupils are equally round, and reactive to light and accommodation. -Extraocular movements are intact. -No ptosis. -Facial sensation is intact bilaterally. -Normal muscles of mastication and facial expression. -Hearing is intact to finger rub bilaterally. -Palate elevates symmetrically. -Sternocleidomastoid and trapezius strength is 5/5 bilaterally. -Tongue protrudes midline without atrophy or fasciculations. Motor exam: Normal strength, tone and bulk in all 4 extremities. Pronator drift is absent. 5/5 strength and FROM all 4 extremities in flexion and extension Sensory exam: -Normal light touch and proprioception in all 4 extremities. -Romberg sign is absent. Coordination exam: -Normal rapid alternating movements, finger-to-nose and heel-to-shin test bilaterally. -Gait: Normal stride, arm swing, turns. Reflexes: 2+ in the biceps, triceps and patellae bilaterally.
